Tools and diagnostic systems in the era of AI

Artificial Intelligence and digitization, i.e. e-health of the knowledge society

Digitization of clinical, analytical and diagnostic trials has already reached a very high level of development. The majority of medical equipment are electronic devices transforming data gathered by means of detectors into digital information. The volume of data serving as a basis for a diagnosis and determining the course of treatment is so large that systems have been already introduced in the 1970’s. The concept of e-health has become synonymous with improving the efficiency of health services through digitization in all spheres: starting from logistics, ending with clinical decision support systems (CDSS) The size of the digital healthcare market can be estimated on the basis of the health care budget. The more the priorities will be focused on the patients’ needs, the larger it will get. The majority of experts’ reports on the healthcare services state that the efficiency and quality of the healthcare system can be boosted through unlocking the potential of e-health technologies and tehcniques. The lists of recommendations on possible reforms are currently elaborated. The development of technology, Big Data industry and AI has made it possible to radically improve the quality of diagnostics and clinical decisions. The combination of the possibilities offered by today’s telemedicine, CDSS systems equipped with AI modules with on-line access to medical data bases, with access to the information in patients’ medical records creates an ecosystem allows for a radical increase the level of medical services and the effectiveness of treatment. The AI technologies have also changed the pharmaceutical landscape. The role of AI in the development of drug syntheses is also growing. We are witnessing the introduction of the first robot scientist, the role of which is to design and test new drugs. IBM Watson Health is a great example of how future artificial doctors can look. First tests show that in the near future AI-equipped machines may be used in everyday clinical practice and draw from all the information gathered by medical sciences. The level of development of digital technologies employed in medicine raises the need for serious discussion on on organizational and legal changes, which must be made in order to fully benefit from the achievements of science and technology in this field.

The main thesis of the panel discussion
In order to fully use the potential of e-Health, thorough reforms are required. They are a prerequisite for raising the general level of knowledge of citizens – patients Only on this basis will be able to build a new culture of relations: e-patient – e-health – prevention – the pharmaceutical industry -digital industry.

Description of the issue

Contrary to the common  opinion, the problem of health care reform will not be solved through pumping more money into this market segment. Radical organizational and legal changes are needed. They require professional competence that is impossible to find in the healthcare segment. Within the current decision-making process it is not possible to make informed decisions on the basis of ready-made and well-known recommendations coming from industry, because the reforms in the area of health care can take place only with approval of the environment. The same problem has blocked reforms and progress in the energy sector. This problem is faced not only by Poland. The same obstacles are faced by the majority of developed and highly developed countries. In order to solve this problem, we must combine medical knowledge and the knowledge related to digital technologies and process management methods. However, it is notenough. We have to develop a scheme of evolutionary business transformation, based on raising qualifications and generation exchanges, which will help us to avoid mass redundancies and the general disapproval of the next big change. Given the above, it is possible to elaborate a list of priorities and actions that will help to set the Polish healthcare system on a path of digital reforms. The main priority is to raise the level of popular knowledge on e-health.

We need a digital curriculum and exercises in the use of the core e-Health systems. For example, it should be taught how to start a personal medical record and keep it up to date. A way to ameliorate the qualifications of the staff is to open e-health higher education facilities. The facilities should train IT staff responsible for logistics and physicians-IT specialists that will be able to elaborate new methods of implementing AI and Big Data technologies in clinical practice and e-prevention systems.
Another important action is to create data centre networks used for management of national system of medical records and medical data bases. This should be perceived as the basis for development of AI modules.
The third action is to connect facilities conducting analytical tests to medical record management systems, so that all the results of the tests performed by a patient in public or private healthcare facilities are recorded in the patient’s medical record.
Nevertheless, without important changes of the general perception, it won’t be possible to launch the necessary reforms and enshrine them in the law. That is why, launching a patient-friendly e-health system is a great educational challenge.

What we need are discussions, questions, answers and schedules:

  • Training of physicians specializing in medical IT

what kind of actions should be taken in order to make it possible for Polish universities to train specialists on Big Data and AI, aware of the needs of an e-state? How to re-organise the administration in order to establish a system of continuous training and decrease staff fluctuations?

  • Development of Big Data analysis and self-learning software for medical purposes and prevention systems

What kind of legal landscape is required to introduce digital medical records? Are we ready to establish digital environment for e-state officials? Does  the development of systems and e-services require any special interoperability framework? If so, how can we define it? If not, what kind of interoperability rules should be developed and what kind of services and systems should be elaborated?

  • Construction of networks, data collection infrastructure, and knowledge required by the development of artificial intelligence

Clinical centres for data, information and knowledge? Development of multinational centres for medical knowledge?

  • New boundaries of responsibility and business models

What kind of laws and duties an e-patient will possess? Should we introduce digital healthcare facilities?
